Community Health Project Paper: Part One
NURS 492: Community Health Nursing
Purpose of the Assignment
The purpose of the project is to examine a community health problem for a specific aggregate. The project is to be able to apply the nursing process to a vulnerable population within your community. In addition, the purpose of this assignment is to utilize the Health Planning Model to improve aggregate health and to apply the nursing process to the larger aggregate within a systems framework.

Approach to the Assignment
The way that I approached this assignment was by choosing an aggregate that was related to the specialty that I am involved with, so I chose the local Pregnancy Resource Center. This facility needed a lot of help, and I am so glad that I was able to lend a helping hand. I immediately set several plans of action into play, but ultimately landed on the lack of community outreach, social media, and nurse taught classes. I was able to contact many different apartment buildings in the area and personally deliver a vast number of flyers. I was also able to update the social media page and implement several nursing taught classes. Ultimately, I was able to utilize my time here and really impact the community.

Reason for Inclusion
I included this paper to highlight my leadership skills and community involvement. This was a productive project that led to increased professionalism, communication, and leadership. Being able to assess a community outreach and formulate a plan of action for improvement and see it through was an extremely rewarding process. While I did implement several new ideas, I also outlined several new plans of action for future use.

Teaching
Uses information technologies to communicate health promotion, risk reduction, and disease prevention
Via the use of social media, flyers, and nurse taught classes which were designed online a communication of health promotion not only during pregnancy but also in regard to risk reduction involving sexually transmitted diseases (STDs) were communicated. Appointments for promoting health with prenatal vitamins, ultrasounds, and classes were explained, reducing the risk of acquiring STDs were also outlined and informationally informed within posts and flyers both paper and online.

Leadership
Assumes a leadership role within one’s scope of practice as a designer, manager and coordinator of health care to meet the needs of populations
This project allowed me to assume a leadership role within an aggregate of my choice. I chose the local Pregnancy Resource Center and designed, managed, and coordinated a new means of outreach for the community. This included flyers, social media, and the introduction of new nurse led education topics. The information relayed includes men and women and an array of services that are provided to the area.

Initiates community partnerships to establish health promotion goals and implements strategies to meet those goals
I was able to initiate new community partnerships with many apartment buildings in the surrounding area. This partnership led to thousands of flyers being dispersed within new tenant packets and being hung in the lobbies as well. The health promotion goals include healthy pregnancies and the prevention of sexually transmitted diseases. Strategies outlined within the flyers included ways for prevention and also screenings and testing.

Organizes, manages, and evaluates the development of strategies to promote healthy communities
I organized, managed, and evaluated the development of strategies to promote healthy communities by personally calling and distributing flyers. I also organized, managed, and evaluated the development of the social media strategies. This included creating posts, and evaluating the outcome such as likes, interaction, and comments. As for the nurse led teaching classes several topics were discussed but required further planning and funding that is still in the works.

Culture
Maintains an awareness of global environment factors that may influence the delivery of health care services
I made sure to address the global environment factors that may influence the delivery of health care services such as lack of transportation and low-income status. This was heavily prevalent within the Radford City area of which was my main focus. I also noted how this is the only resource center to serve the entire New River Valley which would tie in transportation to be a potential factor of influence.
